Name Field

  • Collects first and last name in separate boxes
  • Appears as one field in Google Sheets
  • Use text field for single-box name collection

Basic Information Fields

  • Text: Single-line responses
  • Email: Valid email address collection
  • Phone Number: Auto-formats based on region
  • Text Area: Unlimited character responses
  • Website: Includes "http://" prefix
  • Date: Region-specific formatting
  • Time: Adjusts to visitor's timezone
  • Currency: Customizable currency symbol
  • Number: Numeric-only entries

Selection Fields

  • Dropdown: Single selection from menu options
